The medal is suspended by a ring through a suspension loop to a standard Soviet pentagonal mount, covered with a silk moiré blue ribbon featuring three narrow white stripes along the edges and one in the center. Ribbon width: 24 mm.

Obv.: A rescuer pulling a drowning person to safety in the water.
Inscription along the top:
ЗА СПАСЕНИЕ
(For Rescue).
Inscription along the bottom:
УТОПАЮЩИХ
(of the Drowning).
Rev.: Hammer and sickle above a laurel branch and the inscription:

(USSR)
Both sides are bordered by a raised rim.
This medal was awarded for:
– personal bravery and self-sacrifice during rescue efforts on water;
– vigilance and resourcefulness that helped prevent accidents;
– exemplary organization of water rescue services.
It was awarded by decree of the Presidium of the Supreme Soviet of the USSR and worn on the left side of the chest, after the medal “For Courage in a Fire” when worn with other Soviet decorations.
